Welcome to the Solara Preferred Provider Program, where Chickasaw Nation employees and their covered dependents with coverage under the Chickasaw Nation Health Plans can receive authorized surgical and diagnostic services at zero out-of-pocket expense when performed by Solara Preferred Providers at a Solara-associated facility.

No referral or voucher form is necessary.
